If someone has a problem due to a prostate condition or due to not drinking adequate water, he may experience difficulty while passing urine. I know of some people with such problems strain themselves more than others, and this leads to their taking support by holding on to the wall.
Urination is a normal body function. It doesn't need any extra effort in a normal person. It is a passive act. Hence some people just let nature do its job by closing their eyes and think of other things when they pee. I am told that in some Eastern cultures, this is a recommended practice but am yet to check on it.
Why have you asked this question twice?

The forcefullness of their urine stream causes those reactions...the stronger the urge to go, the more likely you'll see them brace themselves.
