I have answered this question maybe thirty times so by now i am quite certain this is the Best Answer:
Six years ago while playing golf House developed an aneurisn in his right thigh which cut off blood supply to his right leg killing the muscle and causing him excrutiating pain. He went to hospital where they took to long to treat him so he stabbed himself in the leg with a shot of morphine. They took him for a drug addict looking to score a buzz so they put him on observation for three days. During those three days nothing was done about the aneurism and muscle continued to die affecting the kidneys. It was at that point that House, a kidney specialist, told them to check for muscle death based on what he had seen coming from his catheter. Cuddy had to tell him that she was sorry that the condition had gone untreated so long but that he should have his leg amputated at that point. House, wanting to keep his leg, told her to repair the aneurism and he would just have to take his chances with the pain from all the backed up potassium ond other minerals and contaminents held up by the aneurism. When the vein was opened the potassium killed House with a heart attack. They brought him back to life but he was still in a lot of pain. Cuddy wanted him to have the leg cut off or at least the dead tissue. House chose to be put in a coma instead to sleep off the pain. The minute he was under however Stacy, his girlfriend/medical proxy, signed to let Cuddy remove the dead tissue in his leg knowing that he would never forgive her. He never did. The surgery left House a cripple with some pain and the aneurism being allowed to last as long as it did led to the cronic pain he suffers from. They leg is getting worse instead of better and the pain is getting greater not less. Going cold turkey for any length of time will probably result in him having yet another heart attack and Cuddy and Wilson should know this and easily be able to justify his drug use.
As to why he can not fix it. He didn't approve the surgery. He can't undo the surgery. There is a huge chunk of his thigh missing. If it weren't for that surgery, things would probably have healed themselves by now with some dialysis, maybe a little plasma foresis, and a blood transplant or two. Basically, I am thinking he needs a complete lube job because now, not only is his circulatory system shot but his nervous system is trashed as well. A nerve stimulator might help him but it would preclude him from doing his job. Of course, his condition could be worse than that. In which case, fixing it will only be temporary and will probably make it worse than it already is.
Also, he is a drug addict and was a drug addict before the leg thing. He likes the drugs.

Funny you asked this, I'm watching House right now. Even if this is in the wrong section, I'll answer it :P He suffered an infarction in his leg, and eventually the dead muscle had to be removed. This resulted in partial loss of use in the leg, therefore he walks with a cane. Confusing? Be thankful I went with the 'long story short' method. =D

I think he had a blood clot in his leg, surgery was required and they recommended amputation which he refused, so they took out muscle which left him with a bum leg and constant pain--which is why he's addicted to Vicoden. In a recent episode he was shot and they put him into a drug induced coma (and maybe some sort of brain surgery--not sure on that) that for a short time caused the pain to go away. It's back now, and he's heavy into his Vicoden, again. Anyway--it's a great show!! I love Dr. House!!

I love that show. The answer is that i think he got a clot in his leg. He was put into a coma for his pain, and his wife made a desision against his will (because she had his consent since he was in a coma) to do some sort of surgery. This then made his a cripple. That is pretty close to the real thing. I think the episode of when that happens is somewhere in the second season. Hope this helps.

He had a muscle infarction in his right leg. Which caused the muscles to die and makes him have constant pain. It took 3 days to be diagnosed. To make a long story short, he did end up having surgery to fix it and it worked for about six months. Then the pain came back.

There was an episode that talked about that, but I don't remember it very well. I think he had some kind of illness, and refused treatment for too long, and it did permanent damage to his leg.

House had some sort of accident. It was covered briefly this season or last. His co-workers believe his leg pain is psychological in nature.

They did an episode where they explained that. Watch for it in syndication. The actor who plays House is British and puts on an American accent on the show.
