Actually, they use real cigarettes. Most of the time the person is
already a smoker.
Personally, I always ask at casting if the actor / actress is a
smoker. If they are not, I make sure that any action line that
indicates that the actor smokes is removed.
I wont ask a person to do something they don't normally do.
I should also add, I am an writer / director.

Most use a fake cigarette and effects are added to make smoke appear like it's coming out of their mouthes. If they actully made an underaged actor/actress do that, the director would probably be sued or arrested.

To be honest, I think most underage actor/actresses already smoke. Look at Drew Barrymore. She was already an alcholic by age 11!! Ha. Anyway...I do think they have a special cigarette. And also, I have a friend who is an actor, and I don't think they inhale. No inhale, no addiction. Probably tastes awful for them if they don't already smoke. Just my take on things.

Producer has it right. They use real cigarettes, but unless smoking is inimical to the script, it can be deleted.
