Because it isn't under fisticuff, it is under fisticuffs.
It literally means a cuff (blow or hit) with the fists.

I know, it is silly to have to worry about the "s", but the search engine didn't show anything until I added it.
By the way, there is also an interesting sounding band out there called the Fisticuffs. They have a listing in Wikipedia.
Now you know.
