It means that an argument has escalated to fisticuffs. In other words a physical fight has broken out.

To go beyond the point of shouting, and actually hit each other. It was used in the form, that a situation would rend in a fight = it would come to (become) blows (physical).
To come to blows, cam be used when describing individuals who engage in combat; to fight; -- said of, armies, and nations, too.
It was a nautical term, for to c'come to' or to luff; was to bring the ship's head nearer the wind; a position that would be used when firing the guns at another vessel chasing. Thus the persued ship would come to, to return the fire, from a steady ship- but only as a last resort, because the persuer had the wind gauge, and therefore the advantage - thus it was done when no othr option was avliable.
