The only thing that will make the bathroom smell bad is if you do not flush or if the vent pipe out through the roof is blocked

The answer is no. Majority of houses in Norht America have their bathrooms inside and most if not all have exhaust fans.

bathroom placement has no effect on the smell. just make sure you flush regularly and even use the sink and tub regularly so the traps underneath get flushed out.

Clean your bathroom with disinfectant cleaners, empty trash and that will keep it clean and germ free. Open the window occasionally if you have one - it helps.

No they do not as long as you do not have a cold air return in there.Maybe a exhaust fan just in case.
