No, he didn't. His casino empire did file for bankruptcy which is different from him personally filing for bankruptcy. A bit of a conundrum in terms, I suppose.

Not personally.On Nov 2, 1992, the Trump Plaza Hotel was forced to file a prepackaged Chapter 11 Bankruptcy protection plan after being unable to make its debt payments. Under the plan, Trump agreed to give up a 49 percent stake in the luxury hotel to Citibank and five other lenders. In return Trump would receive more favorable terms on the remaining $550+ million owed to the lenders and retain his position as chief executive, though he would not be paid and would not have a role in day-to-day operations.

He never went into bankruptcy personally. One of his casinos that he was CEO did.

No he did not but his company had serious debts and the banks were after him, but somehow he did overcame it, probably by getting some new investors, plus the RE had a big boom in NY, which helped him a lot.
