I notice that the water is being pushed out the caulking below the pipe at the tub and surround seem. I put some pipe dope on the fittings both from the wall and where the spout screws on to the pipe. Would just a bad spout cause this to happen?

Yes you can try a new spout, however my experience is that will not be it. First when dealing with water under pressure always use tephalon tape and pipe dope. Be careful with the tape when tightening metal into plastic though. The leak is most likely behind the tub wall. If you can see up behind the valve you may be able to see water coming off the valve or pipe or the 90 that the shower arm threads into. If you can't see anything, take off the trim plate behind the handle, run the tub and the shower and see if you get water behind the wall. Usually when spouts are bad they just don't work. Also there is a small hole behind the spout (you would have seen it when taking off the spout) use a silicone caulk and close off that hole around the pipe coming through it. Give it a couple of hours to dry and put the spout back on. You should do this regardless what the problem is. Good Luck

prior answer is wrong Yes spounts can break if over tightened and/ or leaned on. A leak from the wall could show the same thing but if you have already replaced it once and pipe doped it I would assume it is the spount. Some have diverters in them for the shower that can also wear out and cause minor leaks.

coming out the caulking indicates the leak is behind the caulking, in the wall. Need to get in there and check it out...might be a $$$ plumbing job.
