Do you have any corporate info? You might be able to find it online. I'd go to the top if necessary-that's ridiculous! Perhaps if you raise enough of a fuss, they'll mail you the refund, but if you send your phone back, make sure you get delivery confirmation and document every single phone conversation you have with them, making sure you take names. If you're persistent, you can win this battle. Good luck.

Assuming you purchases a phone and plan from a provider such as Verizon, Sprint, etc. go to a local store and ask to speak with manager. If that does not work contact the company directly.

File a complaint online with the Better Business Bureau.
File a complaint with the Consumer Protection Agency.
Contact your state legislator.
Always keep a record of the person you spoke with, what was said, where it was (in person, on the phone, what?), and what if anything was promised to you and the date.

Look up the public utilities commission in California, probably file a complaint online.
If you know anyone who holds a political position in Cali, have them call....they jump for the lawmakers.
If not use your family attorney, or for that matter any attorney and have his secretary write one of those "You have left us with no choice but to...." letters....That got Tracfone to refund my money, comp me a free phone with 1000 free minutes, however it took months as they were not in Texas where I am but in Florida.

quietly and calmly explain this to the first person you get at customer service, and then the supervisor, and as far as you can get in their system, then threaten to raise a stink with the BBB, the CA AGs office, the FCC, and any other set of initials you can think of...but always calmly and politely.
