I truly believe that if you call the main office in you area and tell them your circumstances, they may be able to give you some kind of help,
But if not have you got IHHS, in your area they will shop for you

There are many agencies that will help you out with buying your food. There are even grocery stores that will deliver your food once a month for you as well. You just need to call them and ask. Schwans is very expensive, and I think you need to contact someone else for your food. I do believe that Visiting Nurses will even do this for you.

Give their customer service department a call or ask the driver when he delivers your order to take down your information & have a sales rep contact you.

Just ask him straight forward, explain your situation, and just try not to just side tracked; make it short and simple. Make sure he knows that you would be thankful for it.
