I don't think she would be happy about it.
But, if you told her gently, you and your best friend could most likely be friends again. I'm not positive though.
It sort of depends on whether or not you two were going out secretly, not secretly, or not at all.

I dont think it'll be such a problem with that, the problem will probably be abpot why you didn't tell her before. But i'm sure she'll be okay that you two are together cause then she'll know her brother is with someone she can trust and vica versa. Just tell her, it's gonna come out sooner or later.

I had a friend a long time that her brother and I had casual sex for three months and when she found out she never forgave me.
If it's true love, then I would tell her you two are in love.
If it's not then break it off before she does find out if you want to keep your friendship, that is.

She really hasn't to say anything, is her brother, not her boyfriend and her brother is free to sleep with who ever he wants to. Besides she's not his mother either. What can be better for her than having her best friend as her sister in law?

Not that it is any of her business, but many of you females think it is betrayal, if you don't tell your friends "who is doing you". I personally don't trust women with confidential information, unless she is my wife. She will probably be upset because she might think you used her to get to her brother, or because she was too blind to know what was going on between two of the closest people to her. That's my speculation. Who knows, she might just take it like a pro, or may actually already know!!

If she is your best friend, why have you been keeping this a secret from her? Obviously...she's not held in very high regards with you. Now it'll probably hurt her even more.
