I'm trying to make a card for my Spanish teacher for Navidad, and I can't find it in my book. Please help me!
Or of you don't know what form it should be in, could you please tell me what the verb itself is?

GOD BLESS YOU ALL!!! (or not, if you don't believe in God)

Ok, many people got this wrong. Disfrute is command form for usted. Very bad ESL teacher, you have to clarify. The verb itself is disfrutar, so depending on what you are trying to say, the correct form might be "disfruta" as K M said.

Whoever said "espero sea de su agrado" was good but be more correct and say "espero que sea de su agrado" (I hope it is to your enjoyment, a bit redundant, but correct grammar at least). Or you could say "espero que lo disfrute usted" (uses subjunctive, means I hope you enjoy it).

What I am guessing actually is you are trying to say to enjoy yourself? "Espero que usted se disfrute mucho por su vacacion de invierno." (Hope you enjoy yourself alot this winter break). Hope this helps. Don't forget accents (which I did not include because this computer is no good).

Sources: Bilingual, I actually speak Spanish.

Imperative like "Enjoy!" : Disfrute! or Disfrutelo!(Enjoy it!)
Regular: like , "Do you enjoy?" Disfruta Usted?
Past: like ,"Did you enjoy?" Disfruto Usted? (with accent over "o")
