I would guess from the question that you are in Elementary School. If so, some of the references will not be that helpful since they will be more advanced than you need.
When you first start writing book reports you need to think of them as a way to tell the teacher or class what the book was about. "I thought it was interesting" is not a book report.
Start with the basics - the title of the book (make sure you spell capitalize it the same way the book does) and the full name of the author. Also your name so the teacher can give you credit.
Then you need to think of what impressed you about the book. You should write a brief summary (a sentence or two) describing the book in general. You could continue on from there to write a "who, what, when, where" summary but it would be better if you wrote more from the heart. If you liked one of the characters, write about the characters in the book and how and why you think they acted the way they did. If you liked the plot, describe it and don't forget to tell your feelings about the twists and turns of the plot. Even if you were impressed by the place where the book took place or the time where the book was set, you could write about those.
The important thing is to find something in every book that you enjoy and to put that enjoyment into your book report.

The main thing in a book report is summing up the story. You should have a section that includes characters, one for the settings, one for conflict, one for the resolution, and one for a general summary. For example:
Book: Cat in the Hat
Characters: Boy, Girl, Cat, Fish
Setting: A house
Conflict: The cat makes a mess of the boy and girl's house
Resolution: The cat cleans it up
Summary: A boy and a girl are at home on a rainy day. A magical cat comes to entertain them. Their fish tries to tell them not to listen to the cat, but they do anyway. The cat ruins the house while they are having fun. The cat cleans it up.
That was overly simplified, but you get the idea. Your teacher may also want you to do a display of some sort, which I would recommend using paper mache' or a shadow box for. You can find the instructions for these on the internet.

It depends. What grade are you in? Usually a book report requires you to read the book, then sum up the main points, talk about the characters and how you felt about them. Did you like the book? Why? Would you read others by the author? Was the story fiction or non-fiction? Was it believable? and so on.

First you read the book. Remember passages that you think are important, if you can. Make notes if you need to. When you're finished the book, think about it's meaning and summarize the story on paper. Depending on the level you're at, you may need to quote from the book and explain the relevence of the quote, and how it helps to explain the meaning.
