On the part where is says "IF YOU ARE UNDER 18 YEARS OF AGE , CAN YOU PROVIDE PROOF OF ELIGIBILITY TO WORK"? The choices for that are yes, no, and not applicable. I'm 17 years old what is the "PROOF OF ELIGIBILITY TO WORK"? Is it my green card or is it something else? Also if it is something else where do I get it from? I'm Trying to get a job a Carl's Jr. And my school is currently closed there is no one there. How can I get a job before my school opens. I also live in Los Angeles California.

Your birth certificate, baptist certificate, medical records, driver license, and social security card. You can get one for job purpose only, no benefits entitlement.

i just said yes to those, but then again, i'm over 18. but i've seen that question for applications for people over 18. i think they just want to see if you're a legal US citizen. i've always said yes, and my employers have never asked to see any proof. but i'm assuming it's just a social security card or a green card if you are an "alien".

Say yes, then you can get a work permit, you can still work things out even if your school is closed.
If you say no that means you can't get a work permit and all of that. If you say no you wont get a job.
