On the part where is says "IF YOU ARE UNDER 18 YEARS OF AGE , CAN YOU PROVIDE PROOF OF ELIGIBILITY TO WORK"? The choices for that are yes, no, and not applicable. I'm 17 years old what is the "PROOF OF ELIGIBILITY TO WORK"? Is it my green card or is it something else? Also if it is something else where do I get it from?

It is a green card, if applicable or a birth certificate, or a social security number

Depending on the state you will need a work permit and your green card. Just check "Yes" and the employer will let you know what documentation they need. If you check "No" you won't have to worry about being employed.

Edit: You likely won't get a work permit until the Holiday is over, but give the school a call anyways just to check.

Many areas require "working papers" for minors, defined as certificate of age for the state labor department, demonstrating that a minor is older than the age considered to be the child-labor age for a particular occupation. Call the local office of your state labor department for more information.

They need a work permit. You get these from the school guidance counselor. If you are no longer in school, you will have to go to your old high school and get one from the counselor there. Good luck!
