I agree with Stephan somewhat. Although the likelihood of his clan actually being alive is slim (Sasuke saw their corpses), it is possible that Itachi might turn out to be a good guy. If that were the case, Sasuke's entire life would have been wasted since he's devoted all his energy to trying to kill Itachi. Other possible nightmares for Sasuke would be the death of his BFF Naruto or finding out about all the NaruSasu fangirls out there.

Aside from what Stephen said, Sasuke worst nightmare would be if Itachi wasn't actually strong as he comes out to be. Imagine if Sasuke laid one blow to Itachi.
Here's my idea/script:
Sasuke and Itachi are having their duel in their old village. Sasuke moves super fast and punches Itachi on the chest.
Itachi: Damn...
Sasuke: That's what you get Itachi. You bastard...
Itachi: Ouch!! Why you have to do that Sasuke? Crap this hurts!!
Sasuke looks as Itachi falls to the floor crying in pain.
Itachi: Awwww. Mom, ahhh. Damn, why did I have to kill everyone. Sasuke, you just owned me. Will you ever forgive me?
Sasuke: ....

After he sells his body and soul to Orochimaru to kill his brother and finally kills Itachi, his whole family pops out of nowhere yelling "Surprise, it was an extremely long, ardurous, and elaborate prank!" And it turns out that Itachi's not really that bad of a guy.

Oh my gosh, Stephan, that would SOO suck if that would actually happen.
Most likely, yeah, it's his brother. And maybe death itself too. I mean, it was death that took his entire Uchiha clan and what keeps him awake at night...or something...
