The difference is very subtle. "help me" is more general and can refer to any situation where help is needed like if you are stranded on the road side on laying sick in your bed. For example if you were sick and you went to your doctor you wouldn't say "help me out" you would say "help me" Now if you were down on your luck and needed some money you would say "help me out" The situation clarifies the meaning. Both can be used at certain times but sometimes you can only use one or the other.
