You would contact a civil attorney.

And check your records to see if you may have accidentally signed anything or you are in posession of anything (relating to this company or product) that says something to the effect of : "By buying our product and notifying us about it's effects, your name and likeness can be used in anyway, including, but not limited to, advertisements for the company and producut." It could even be a 'disclaimer' on their website and you clicked an "I Agree" box without reading it fully. Anyway, It gives them consent (aka permission) and is usually in the fine print.

By the way, are you sure it's your name? It could be a namesake! I know I have a lot of those!
