Well, when I was a kid we usually play doctor at school too and it was fun. And I don't think there's something wrong about doing that and I think that it is also good for children to play doctor so that as they grow up they will learn how to interact with other people and how to handle such situations like accidents and stuffs.

Yes, very normal. They need to be instructed that it is not appropriate, if it goes too far, but not punished as if they did something wrong. Always leave them comfortable to tell if someone is doing that to them.
Judging by your picture, they are probably learning a lot at home.
