A letter of resignation does NOT mention the name of the new employer because it's not relevant. The letter's purpose is to simply let them know that you'll be leaving as of a specific date, and your reason(s) why.
Now if you have a good relationship with your boss, you can tell him or her where you're going, but it would NOT go in your letter of resignation.

Up to you. If your leaving on good terms and are not concerned about a some one at your old company being vindictive. personally i wouldnt. They have your mailing adress, Thats all they need.

No reason to, you don't really owe them anything. Sometimes if you need a reference you have to tell someone, but you already have the job. Congratulations.
