The answer varies because different types of procedures have different requirements. You will be told how undressed you must be under that gown. If those instructions aren't clear to you, ask your question. Take shorts with you just in case.

It depends. I had an emergency room visit a few years ago and had to wear the gown, but was allowed to keep my underwear.
I had a couple of hospital stays a few years ago and at first I had to wear only the gown but after a few days was allowed to wear underwear under the gown.
It depends a lot on what you are there for.

Sometimes the Nurse Practitioner or Doctor will inform you if you can leave any of your clothing (bra, underwear) on underneath the hospital gown. You can tell them you are uncomfortable being naked under the hospital gown and ask if you could wear at least your underwear. They may say no, but it won't hurt to ask!

Depending a on what you are there for.But first ask your doctor in advance,because he will be the one to decide what you can or can not wear.DO NOT wait until you go into the hospital,because the nurse will tell you you have to take it all off.Even if you need finger surgery.That's what they do because they don't always know at the time what proceger you are having and they have to protect there butt if they get yelled at by doctor.But if the doctor in advance says that you can leave on under ware or shorts,then when you go to hospital and nurse says take it all off,you say I talked to doctor and he said you could leave on what ever.And the nurse won't like that,but that's tuff,because you asked the doctor.Good luck.

if you are going into the operating room for this procedure then you are not allowed to wear anything but the gown they have provided for you in order to keep things sterile.
the hospital should have pants for you to wear (made out of the same material as the gown is). so ask for them when you go in.
