I have worked with Fair Housing in California. I got info. from them on the phone when I was renting my condo and they helped alot. I also called them when I was having trouble with my apt. lease.

I suggest that you look in the phone book for their number and try to get info on the phone. They even sent me printed info in the mail. I'm sure they would send you the proper paperwork to file whatever claim you wish to file.

Otherwise, you can look on their website. If you don't have a computer, go to the library and log on to the internet. Go to a search site like Google (a librarian will be happy to help you) and type in Fair Housing and then press search. You should be able to find some needed info on their site. This might take some time cause it is kind of like doing your income tax with the IRS forms. You have to concentrate. If you get the right person on the phone at Fair Housing, that could be the end of your search. If you still need help, call a lawyer. If you can't afford a lawyer, call legal aid.

Good Luck
