This is a easy ? And Im a plumber of 20yrs and your answer is
YES IT DOES.
The shower head should be changed.
As for the water hurting your eyes depending on what kind of pipes you have in house they could bad (Galvinized pipes) Rust from the inside out and will (Could) bother your eyes
Also if its city water They could be adding to much chemicals into the water and that will do it also.
It could be 1 or both of them.
Call the city water Dept and ask they to do a test on your water (ITS FREE) if it coes back good then its your pipes.
almost forgot it could be your HWT if its old they could be alot of buildup of dirt/crap in the tank and when you use the hot water that dirt and crap will come out.
Get a Glass and turn on your hot water only ina faucet let it run till its HOT then fill the glass up and look through the glass for any dirt or anything.
Also this is a maintiance thing for your tank on the boittom of your tank there is a valve (looks like a garden hose hook up) Hook a hose up to it and run the hose outside turn it on and let water run out till its clear then shut it off.
Most people think they dont have to do anything to there tank but this is not true with anything there is always something to do.
NOTE-- Sometimes not all the times but if the hose spicket on the HWT when you open it up they dont like to shut off cuz they have never been open in yrs if this happens you will have to change it (its not hard at all) If this happens to you drop me a line at

ANd I will take you through it step by step

Kos has a good answer. I'd like to add that shower filters are available from Home Depot and elsewhere. You have to remove the shower head, screw the filter onto the gooseneck, and then screw the shower head onto the filter. These will remove chlorine from the water as well as sediment.

it never hurts to take off the shower head and clean it if it looks that bad. It might hurt your eyes because of that or the water pressure do you have sensitive eyes / normally wear glasses.B-)
then it could be that. But just take the head off clean it with whatever bathroom cleaner ya got. Then rinse it real good, before and after you put it back on let the water run to rinse the inside of it.

remove and replace the shower head. It's probably the galvanized pipes that color the head, and stop up some of the holes.
