Lead chlorate = Pb(ClO3)2 This assumes that you mean lead(II) chlorate And not lead(IV) chlorate. How to identify it? If there were no possibility of another metal, then adding a sodium chromate solution (Na2CrO4) should result in a dense insoluble precipitate of lead chromate.

The formula is

Pb(ClO3)2

Those numbers are subscripts.
