Vanity is part of a self-esteem problem. It is mask to hide perceived inadequacies. They may not even be known to the person on a conscious level. It is a rooted emotional issue.

YES! Usually people who are vain have self esteem issues. That's why they cover it up by being all about themselves to other people.

i do think that is very possible, i think that some people put up a face like they are vain, when really they are very self conscious. i think that for some people, if you pretend to be confident and love yourself, it makes it easier for them to cope with the fact that they are actually not. like me, i act confident and make people think that everything is perfect, but when i am alone, i know that it is not true, in fact the true me, is really quite different

If their reason for obsessing about their appearance is driven by insecurity, "How do I look now?" "If I look less than perfect, people won't like me." then of course they would appear vain. The behavior would be driven by insecurity and not by narcissism, "Boy, don't I look great!"
