I know there are many carpet price ranges, but can someone ballpark how much per square foot the "middle" grade of carpet costs with installation - I don't know how to install myself.

Mid-grade carpet $12.50 sq. yd
6lb rebond pad $4.00 sq. yd.
install 3.50
Carpet is sold in Square yards, square footage is a gimmick used to make it appear cheaper. Home Depot and Lowes do it, but ask around and you can find a better more reputable installer than one that they will provide. Padding and installation prices are usually give in sq. yd pricing.
length X width divded by 9 will give you the square yardage of a room.

Cheap carpet apt grade is $1.30 to $1.69 sq ft
Base grade called upgrade by a builder Is $1.69 to $2.00 sq ft
Good grade is $2.00 to $2.50 sq ft.
Better for medium to heavy traffic is $2.50 to $4 sq ft..
Best carpet for heavy traffic is $4 up. Some special look and design carpets may be more. This is for carpet, pad, basic install labor. May be higher in some metro areas and high freight cost areas. Prices above assume buying on sale prices. National average is $3.70. Expect to pay more for steps and furniture moving, removing old carpet, other custom work.

per square foot on some good mid range carpet you should pay about $2.50-$3.25 and don't try to install it you're self some things are better left for a professional

The type of carpet you are using has alot to do with the price. Talk to several contractors, and get written bids. Don't skimp on your pad. The pad has alot to do with how well your carpet looks and wears.
