I am doing a survey to rate tenants and i need questions. For example
Did they Leave the house in reasonable condition?
Has Anything been broken/damaged?
Did the create excessive noise?
Was rent paid on time?
Any others?

Boy, is that a loaded question.

A bad tenant is:

1. Doesnt pay the rent on time
2. Doesnt keep the utilities paid up
3. Has constant complaints against him/her for multilple infractions from other tenants.
4. Doesnt keep the apt clean, so the bugs move in and then its the landlords fault.
5. Thinks they run the building, not the landlord.
6. Never returns phone calls or letters to the landlord.
7. Tries to refuse to let anyone in to do repairs.
8. Tells the contractors how to do their work, and yells if they think its being done the wrong way.
9. Damages? Who me? It was that way when I moved in.
10. Has wild parties until all hours of the night.

Ok, this is a bad tenant. Now to your questions

1. Pay rent and utilities on time.
2 Were you ever evicted or taken to court for damages or late rent
3. How many times did the cops have to come out to their house.
4. Would the neighbors give the tenant a favorable report.
5. Would their old landlord take them back as tenants.
6. Did they get all the security deposit back. If not, Why?

You have GOT to be kidding me, cazmo!!! You were leaving anyway, so you didn't have to pay the last two months rent????

Well, if you want an example of a bad tenant, look no further than cazmo. Here's one to add to your list - not paying the rent at all.

Did they comply with security measeures? In my building, people tend to prop open the security doors so they don't have to use their key, or to let others in. This takes away from the purpose of these doors, to keep strangers out of the building unless accompanied by a tentant.

All the above with some additional facts like having too many pets, selling and using drugs, removing appliances from the apartment and selling them.

ok then
the carpet was cream so of course the stains couldnt be removed
the kitchen units were falling apart anyway
we only partied hard at weekends till the early hours
and we were leaving anyway so didnt need to pay the last 2 months rent

Consistent late night, boisterous, noisy parties with alcohol, drugs, and hot women.
And! They don't invite you.
