Does the xbox 360 premium (20 gb) already come with the patch to play xbox games, or do I need to download it?

The 20 GB harddrive allows u to play backward compatible games only after you download the program from xbox live. For most 360's, the program runs itself.

when you play an xbox game on the 360... it will let you know that you have to download game content. downloading the content is the only way you can play these games.

You need to download it. It will automatically download the required patches when you insert the game.
