He's a great guy to work for, so I wanted to get him a little something.
(just a gift certificate to his favorite lunch spot)
Not sure how to present it:
Do I put my name on it? - Part of me wants him to know how much he's appreciated.
Or Do I make anonymous? - Don't want to look like I'm asking for special treatment.

Go ahead and put your name on it. Appreciate him. Everyone loves to be appreciated. So, go ahead and say few nice words. He will certainly welcome your gesture and kind words.

Definitely put your name on it. If you truly enjoy working for him, let him know in a card and include the gift certificate. I am sure the card will make a longer impression after the free lunch has been eaten.

Put your name on it unless it is for KFC.

Don't let Leonardo DiCaprio take credit for it like he did for all of your work on greenhouse emissions.
