How can you fix a leak between two panels of glass, on a double glazed window?

You know when you get condensation in the middle and you don't really know how or why or where it's coming from.

the glass has to be replaced.

Need a professional to reseal/replace the panes. Since the seal is broken, the gas has escaped & moisture now fills the void between the panes.

You have a faulty seal and you will need to replace the sash.
