This person is probably suffering from schizophrenia, and actually is hearing other voices and sometimes seeing things that aren't there due to a misfiring of neurons caused by some as yet undiscovered reason. This is a terrifying thing for the person, and those around them if not treated and can cause the complete dissolution of the persons family and even death, usually due to not paying attention, suicidal thoughts or not paying attention to basic health and hygiene.

When i am in the store. I talk out loud to myself by reciting the list of things i need. Does it make you crazy that you are talking to yourself inside your head, when you come up with these questions? Or that you are reading these words i write right now and commenting on them inside your head? Everyone talks to themselves. Some people just make it more audible than others. If you are looking for signs that it is a hallucination, a person hallucinating will look like they are having a two sided conversation with no one.

They could have bluetooth technology. You'd be surprised how many people who don't appear all that on top of things actually are perfectly fine - and have bluetooth.

Or, they may have mental illness. People who have schizophrenia do hear voices and are not in touch with reality. They don't "see" the real world as it really is, and they don't hear it as it really sounds. Who knows what's going on beyond that...

Once in a while it could be a person who prefers to have conversations with himself and doesn't care what you think about him if he does.
