i think its for the sewer but im not sure

It's a clean-out plug for the sewer piping. The square plastic part is what you put a wrench on, and the whole plug unscrews so you can stick a plumbing snake in there to clean out the line.
Code requires them to be placed at certain intervals on the lines. Code also requires it to be accessible, but as long as you can get at it with simple tools, like a screwdriver, you can cover it.

Most likely it's a clean out plug for the sewer. Most often used to go from the house to the mainline

the small plastic plug is so that the gnomes that live between the walls can get out at night and steal your underpants. make sure you dont block the plug hole or they will die back there and make a nasty smell as they decompose.

It's either a clean-out plug or there used to be a fixture there - like it used to be a bathroom sink.
