It sounds like you may be obsessing about these things. Generally, this kind of repetitive thinking is due to intense anxiety about whatever it is that you did.
My advice: Ask God for forgiveness. Receive it!
Then, get a couple of sessions of counseling so that you can forgive yourself and move past this.
If there is someone you need to make amends to, and it won't hurt that person to contact them and do so, then you can do so. If you cannot contact the person or don't think it wise to do so, you can give a donation to charity. Do something concrete so that you have the sense that you have made amends.
You aren't alone in having made mistakes and poor choices. Or in having done things that you are ashamed of. You are right to ask for help when you get stuck in recycling the past, though--
either you have some unfinished business to attend to or you are experiencing unreasonable guilt and anxiety. So go do something constructive to address your anxiety and...
Forgive yourself. Accepting that you are doing the best that you can.

I'm fortunate that I have not done many things in my life that fit in that category, but there have been some. I have found that the only way to exorcise those thoughts is to do the following:
1. Acknowledge the wrong and understand why
2. Make right whatever I did was wrong.
3. Make sure to make changes so that it will not happen again.
Until you do all three things, the ghosts of your past will continue to haunt you.
Good luck.

I often think about the bad stuff I have done and feel like I need a good paddling for each offence.

mostly I'm only left with things I'm embarassed about
I haven't hurt people that much that I'm aware of anyway. Except one person if she feels anything like I do about her.
Anyway, you have to deal with it. Apologize to the people you hurt and get their apologies to if you can. Go to church and get absolved, confess to a good therapist. Whatever works.
