The best way is to call t-mobile and tell them your are traveling internationally and are going to need your phone. They will give you the unlock code on the spot. Make sure you are nice to the rep. If they don't want to give you the code, just hang up and try again. I've had numerous friends do this and it has worked for them.

The codes for unlocking phone networks are different for every phone, it goes by your IMEI number and network provider, so a working code for one phone will not work on another phone even if it's the same model. Some phones are also unlockable by code, you will need to take them to a phone shop or a market stall (at your own risk) and pay a small fee and they will unlock them using a specific cable and software.
If you phone your network provider they should unlock it for you aswell for a fee.
