How do I find all this (minus the account name and password, of course).

Yes it is possible.

A lot of mail servers use the mail prefix: mail.servername.com for both incoming and outgoing. Occasionally, they are different. They could be smtp.servername.com or pop.servername.com or imap.servername.com. It really depends on the ISP.

You need to find out from the service provider what server aliases they are using. Or simply try out mail.servername.com in each slot and see if it works.
