Assuming that the battery, starter, and alternator are all fine, what other causes ( if any) would cause a car to not want to start on the first couple of tries?

Poor maintenance in general can cause starting problems.

Dirty Air Filter
Worn Spark Plugs
Bad / old spark plug wires
Dirty fuel injectors
Dirty fuel filter

A dirty air filter can cause your car to run roughly, but not likely to cause it not to start. When looking for problems, always start with the cheaper fixes. You can check your air filter. If dirty it doesn't cost much to replace it or just have it blown out with an air hose. Most drive thru oil change places will do this. The fuel filter could be clogged. This can cause your car not to crank. If the engine isn't turning over, then you may have a more serious problem if the other items you mentioned are fine.

Only under extreme dirty conditions. I have seen some nasty filters on cars that still ran. Most of the time it will colapse the filter and create a leak around the filter. So the answer is probably not.

actually yes, a dirty air filter can cause your car not to start,but it would have to be really really dirty.it would cause it not to start by blocking sufficient airflow into the engine,so that the engine is essentially choked.
other problems are incorrect gap on the spark plugs. if its too high, or too low the spark wont be as strong as it should be in the case of too high, and if its too low,the sparkplug can be essentailly grounded,giving no spark whatsoever.
a bad fuel pump/no fuel. pretty much self explanatory.
clogged fuel filter, prevent flow of fuel to engine,or contaminates fuel.
dirty fuel injection manifold/carbureutor.

it's possible for dirty air filters to prevent cars from starting. To be sure of it, take the air filter off and try to start the car. if it starts then it is possibley the filter if it doesnt start, the check the carb.

It is, like your first answer unlikely. Unless it has done what happened to me. My air filter on my car was so dirty it came apart and was sucked into the carburator. Hopefully that's not your case. Best of luck to you...........

Could be Diesel glow plugs are bad.
Could be glow plug timer.
Could be injector pump.
Could be fuel pump.
Could be a whole bunch of things.......

Is it a BMW, Porche, Honda, Ford, Chevy, Toyota, Nissan, ????
Oh, is this gas or diesel?
What kind of car?
Does it have a carburator, or fuel injection?
What year?

Yes, if you can't afford a filter, at least have someone with an air compressor blow it out.
