This happens many times. Of course, before you leave the boat, you should have filled your BC (bouyancy compensator) with air and taken some breaths off your regulator, which should be in your mouth. In other words, you should know if your tank is on or not before you leave the boat. But, if you don't do that, then you'll find out once you hit the water. At that point, things can get exciting. If your BC is empty, you may find yourself unable to get back to the surface. In that case, you need to dump your weight belt, which should let you get back to the surface. You could also look for your dive buddy or another person, to breath off their octopus or main regulators. Only if all of that failed or was not possible would I try to take my BC off and get to my tank, to turn it on myself. That is doable and is even trained for, but it is fairly difficult. It is easier to just get a new weight belt.

I can't say how often I've had to buddy breath with other divers, due to them running out of air.

I know someone is about to say why jump off a perfectly good boat. But if I did, and the tank wasn't working, I'd probably borrow someone elses.

That wouldn't happen. You check all of your equipment before you get in the water, or even on the boat for that matter. If you are with a dive master they don't just let you jump in the water and swim away, it is a progressive thing. And if somehow miraculously it wasn't working you would get out of the water.

I'd jump back on the boat, duh. If it weren't working then I wouldn't even get off the boat in the first place or I wouldn't get far under water. Preliminary checks.
