There's no form for that. If you were required to file, or want to in order to get money refunded that you have coming, then download the 2005 forms from irs.gov and just fill them out and send them in.

If you weren't required to file, and don't have money coming back, you don't need to do anything about last year's tax filing. If you're going to file this year, just go ahead and do it.

There is no such form. Other than the IRS transcript requested by 4506-T there is no possible way of "proving" nonfiling -- except, of course, an assessment by the IRS that says so, or an order of the Tax Court or a Bankruptcy Court.

Since it's an admission (usually) against interest, an affidavit by you should suffice. An altrenative is to file now, showing that your income was insufficient to require you to file. If that's the case. That sets the statute of limitations running, so it's not a bad idea to do it if there's any doubt.
