How do I order the notes of testimony of a Philadelphia Zoning Board of Adjustment hearing?

The forms I've found on the internet and the phila.gov website for ordering Notes of Testimony are geared toward criminal defendants, which is not relevant for my purposes. Is there a separate form to fill out and where do I get it?

Our municipality has a Zoning Board. Its commission has "minutes" that are public record, not "notes."
