My other half has never visited a doctor since he came her from the carribean five yers ago. what does he need to bring with him in order to get a doctor?

It depends on the surgery's policy for registering new patients and where in the world you are. If you are in the UK the best thing to do is phone or drop in to your nearest surgery and ask what their policy is. I worked in a surgery for 7 years and we didn't require any proof of address. We would ask where you live and if it was in our surgery's area of cover then we would give you a form to fill in. If you have a doctor then try to see if he can register there, that would be the best thing.

Does he need surgery? Normally, you would just make an appt. with a doctor. They have intake forms that ask for medical history, etc. They may ask for an Social Security Number but I don't know if that is necessary legal anymore. Most of my medical ID numbers have been changed over the last few years because of identity theft problems with SSN's being too accessible. The best bet would be to decide which doctor you want to go to, then call their office to ask what is needed.

you need proof of address and identity so some kind of bill and a passport. but make the application with the doctor and they'll tell you exactly what is required.

Hi,first pick your doctor then phone & ask the receptionist what they require.Or go into the surgery & ask.
