Yes! How is the waiter supposed to know that your loud sipping is a call for his attention? For all he knows, you're just being obnoxious and annoying the other customers. Instead, try raising your hand (like in class!) or saying, "Excuse me!" But not too loud.

Yes. It is rude not only to the waiter but to those around you who are trying to enjoy their dining out experience. There are better ways to get your waiter's attention.

I've found that it's better to tear small pieces off your napkin, form them into a ball, and then shoot them through the straw at the waiters head.
It's quieter and sure to get their attention.

-Or you can just say "Excuse me, waiter", but the spitwads are much more fun.

yes, i could see why you might do it if the waiter is taking forever to bring you a refill and you are trying to get their ttention by showing your drink is empty. but i would try and wave or flag him down before doing that. it looks childish to make sounds.

yes i think so. I either wave them down or just sit my glass at the edge of the table for a refil.
