Catherine the Great of Russia didn't cut off her lovers' heads! She may have had a hand (not literally) in having her husband assassinated, but she was known for being generous towards her lovers after the affairs had finished.

Tzars and Imperors were kind of big on head-cutting. That's just all there is to it. This was one reliable way to keep someone from talking too much.

maybe they don't know how to use their head or maybe she asked them to and they refused because of some hygiienic smell or something...(just a joke!)
