I have three small green spotted puffers about 1.5 inch each. One of them (the largest one) has been black most of the time and in tail-up-head-down position for a few days now. That one used to be the most healthy-looking, and rarely turning into black belly. But now, it's in this upside-down position all the time and difficult to manuver in the water. The other two seem as active before and without observable behavior change.

Yes. They're in brackish water, about 4 tablespoon of marine salt per 10 gallon.

Check your water quality (ammonia, nitrite, nitrate, etc) and avoid major salinity flucuations (get a hydrometer or refractometer for salinity measurement). 0ppm ammonia & nitrite; and nitrate lower than 20ppm is what you want to have.
By the way, 4-tablespoons of marine salt per 10gal is NOT brackish (brackish would be more like 2-4 cups of marine salt per 10gal depending on how brackish you want to get).
The salt should be pre-mixed and circulated with a powerhead in a seperate container and not added directly into the tank.
Adjusting the salt level at this point is secondary, what's more important is assuring your water quality and parameters are ideal.

Green puffers do not like each other, they can kill each other easily with their vicious beak-like mouths.
Your water is not salty enough, 4Tbsp per gallon would be better, they also like really clean water so make shure there is no leftover food in the tank and do 30-40% water changes once a week.

he will die quickly in case you dont get him in ideal water circumstances.Thats why hes dieing.The dwarf/and reg observed puffer choose particular water circumstances on a greater suitable PH then frequent fish.Did you analyze previously to procure him? is the puffer in brackish or fresh water what is the ph and salinity level i have had puffers in the past mine would live in fresh water with a high slainity level but in year or so they would die
