This is not the actual cliche'. The real, historically accurate phrase is "Cold as a witches teat (or tit)". The "elbow" has replaced the original term in order to make it more politcally correct.
It means that a witch, who does not procreate due to "demonic tendencies" which stop the God given right of bearing children, will not have a baby to suckle at the "teat". Therefore, the "teat" will be cold as ice.
