Habit substitution. Finding something else to do with your hands, and something else, like gum, hard candy, or a straw to put in your mouth can make a big difference.

Also, substituting the smoke break with 5 minutes of deep breathing and meditation will help. It's the deep breaths and the break from real life that are the most addictive, at least to me.
