I have once. I felt terribly bad. My entire school life (I'm in college now) I had never cheated. Ever. When in the 11th grade, I had to do some sort of side thing for my principal during one of my classes (the class with the teacher I highly respected). By the time I came back to class, there were only about 10-15 minutes left. There was an assignment due, nothing big, only worth about 25 points. Since I was gone the entire class time, I had no clue how to do the assignment, and I wouldn't have enough time to finish it. I didn't want to have it as homework, so instead I decided to copy off a friend...
Keep in mind this was the first time I had ever done this, so of course I got caught. My teacher gave me this look that made my heart sink, and then he said "(My name), I am so disappointed in you". Then he took my paper as well as my friends paper and gave us both zeros. It didn't hurt my grade, I still had an A. But, my friend on the other hand... he ended up with a D because of it (he would have had a C [passing]). I asked if I could take both the zeros.. but my teacher wouldn't allow it. He said that BOTH of us had to learn from this. That was my FIRST and LAST time of copying/cheating off of someone else.

Yes, I did. I was expected by my favorite teacher and speach coach to win the nationals one year in speech, I lost. I could tell he was dissapointed, but he never said so. I was dissapointed in myself, I felt like I could and should have done better.

Yep, I sure did, or so I thought. He was one of my high school teachers, and I was sure that I had disappointed him by my incurably bad behavior. I called him several years after my time in high school; I won't go into the number of years, but they were many. When I talked to him, he chuckled a little as he remembered who I was and was pleasantly surprised by my call. I told him that I felt that I should apologize for being such a bad kid. When he heard that, he chuckled some more and said that he didn't remember me as being a bad kid. Obviously, I hadn't disappointed him, he was very happy to hear from me and he thanked me for calling.

My journalism teacher had nominated me for a respect pin at my school for being "consistant" and a "hard worker." It just so happened that the messenger brought it to me in that class when she was stamping the on-time assignments- the one assignment I haden't finished.
I felt humiliated. And ashamed. The teacher just ignored it.
