According to an in-depth FAQ I found, it appears that Revival Jam has been found as a rare in the "Masked Beast" pack. You will need to beat Mako Tsunami ten times to unlock it.
It has also been found as a common in the "Yugi Pose 3" pack. You will need to beat Lumis & Umbra ten times to unlock that pack.
Also, the Millenium Packs contain all of each card type, so you may be able to find a Revival Jam that way as well. However you will need to beat the Tier 6 duelists twenty times each to unlock them.
Beat Simon 20 times --> All Magic Cards
Beat Yami Bakura 20 times --> All Trap Cards
Beat Yami Marik 20 times --> Effect Monsters
Beat Duel Computer 20 times --> All Monsters
Beat Yami Yugi 20 times --> All Cards
