Dax is the Symbiote (a long-lived aquatic worm-like creature about the 5" in diameter and about 8" long) carried by a Trill host. About 10% of the Trill population serve as host to a Symbiote and it is surgically implanted into the abdomen.
While joined the host and symbiote become a single mind. Many of the memories and experiences of the previous host are retained by the symbiote and passed on to the new host.
In Trill society only individuals that have gone through years of extensive training, psychological screenings, and have reached a certain age of maturity can be joined with a symbiote and there are many rules governing "Joined Trills". One of the rules in particular calls for new host to avoid contact and relationships with individuals the previous host knew.
There are a several episodes that mention the details of Trill society and a few go into the details of the Symbiosis Commission that screens potential host and tends unjoined Symbiotes in networks of underground pools. When a Trill becomes host to a Symbiote it is tradition that their family name is replaced with the name of the symbiote.
At the start of DS9 Judzia is the current host of the Dax simbiote.
The previous host was a male Trill named Curzon who was a Federation Embassabor to the Klingon Empire and helped to negotiate several early treaties along with resolving disputes. Curzon was Ben Sisko's mentor and had taken Ben under his wing not long after he graduated from Starfleet Academy. This is why Sisko often refers to Judzia as "old man" in many of their conversations.
Judzia is killed by Gul Dukat (possesed with the power of the Bajoran Pah Wraiths) near the last seasons of the DS9 series and the Symbiote was placed in medical stasis for transport back to the Trill homeworld.
The USS Destiny was delivering the injured symbiote when it's life signs began to weaken and would die before the ship could reach the Trill homeworld. The assistant ship's counselor on the USS Destiny was a young Ensign named Ezri Tigan, the closest Trill capable of saving the dying Dax symbiote. Ezri would technically be classified as an unsuitable host by the Symbiosis board, because she is too young and has not had the proper pyschological screenings and years of preparation for becoming a host.
Other previous host include:
Emony Dax (3rd host): A gymnast that use to stand on her head to relax and relieve stress. Ezri tried the technique, but it only seemed to give her a headache.
Torias Dax: A shuttle pilot that accidently killed himself when he skipped the standard maintence and pre-flight checks on what he thought of as a "routine" shuttle flight. Torias was married to another joined Trill carrying the Kahn symbiote. The two symbiotes were mated (in a married relationship sense not sexually) during the time their hosts were together. Judzia Dax was in a brief relationship with another Trill named Lenara who was the current host on the Kahn symbiote. The two symbiotes had attempted to continue the relationship they had, before Lenara gave in to the pressures to follow the tradition of
Audrid Dax: A female host and mother to a son named Gran.
Joran Dax: A brief host that was mentally ill and murdered a member of the Symbiosis Board before Dax was joined to Curzon. The Symbisis Board forced the memories of this host to be supressed to hide their lies about the psychological screenings. The Board had been using the excuse that symbiotes cannot survive for more than a few days if placed in a metally unstable host, but the truth was that the Dax symbiote survived for more than 6 months before it was removed. The Symbiosis Commision has been hiding the fact that about 50% of the Trill population is actually suitable for joining rather than the 10% the majority of Trills believe. If this were to ever get out Trill society would be thrown into chaos.
Verad Dax: A male Trill that was rejected by the Symbiosis board and believed that he should have been joined with the Dax symbiote. He steals the Dax symbiote from Judzia and is a breif host before Sisko can stop him from escaping and places Dax back inside Judzia.
Yedrin Dax & Lela Dax are mentioned by name, but little details of these host are revealed.
The Dax simbiote has had 4 previous host (before Curzon) that had children, 3 were as mother and 1 as a father.

The biography of Jadzia Dax has to take two parts; that about the symbiot "Dax" who is around 300 years old and has had a number of hosts over the years, and that of the humanoid Trill woman, Jadzia. The symbiot has a colourful history, having been joined to no less than seven other hosts including a murderer. The host before Jadzia was Curzon Dax, a great mentor of Benjamin Sisko in his youth.
Jadzia was an attractive and brilliant young woman who worked very hard to get accepted for the Trill joining programme. She was thrown out of the programme after a poor report from Curzon Dax during the initiate assesment. She was then reaccepted into the programme in an unprecidented move and finally became the host to Dax when Curzon died.

Ensign Ezri Dax
Ezri is the wayward daughter of the owner of a large mining concern who left her family home and business, much to the disgust of her family, to join Starfleet as a counsellor. While on a mission early in her career, she was the only Trill close enough to Deep Space Nine to be joined with the Dax symbiot when Jadzia was killed. As a result she suddenly found herself a joined Trill, despite never having expected or been trained to be so.
Confused and unstable as a result of the joining, she seeks out Benjamin Sisko who the symbiot believes can help her sort things out. This he does and she soon elects to be officially transfered to Deep Space Nine, despite the issues arising out of her previous host having lived there.

Dax is actually the name of the symbiote living inside the host. The symbiote is hundreds of years old. The symbiote lives inside the host like a parasite, but doesn't have any harmful effects. Her past hosts have been men.
