If he plays electric guitar, get him a DigiTech Whammy pedal- he'll be playing around with that thing for months to come. If he plays an accoustic guitar, get him a Jellyfish pick. It's a pick that can make something like 15 different sounds, depending on how you strike the strings.

If you know what kind of music he likes, get him some sheet music and tabs. Say if he likes classic rock music, get him a book like "The greatest classic rock hits" or something like that. The usually are about $10-15 so its nothing cheap or too expensive and you can get them at pretty much any music store like SamAsh or something similar. But its always good to just ask him, "Hey! What do you want for your birthday? Sheet music? What a fantastic idea!"

Another guitar strap would be cool. No picks let him buy his own. or go to ams.com to get an idea. And then go to any music store to buy.

Something like a capo (which clamps the strings down at a certain point along the neck) would be a nice gift i think. I doubt he would use it much (i hardly use mine) but it's a cool novelty to have one. you can get them at music stores.
Spare strings never go astray either, so perhaps you could buy a set or 2 of strings. Whatever you get I'm sure it will be appreciated by a good friend

buy a good quality strap (15 bucks tops), and then paint some words on it...maybe some quote on the strap that ur friend would like or something. a friend of mine did that for me and i love it because it shows that she actually thought about me rather than simply buying something that says "here you go, i didnt know what ud like, so i just got you something decent"

Get him a soft-shelled/hard-shelled case for his guitar. It's a soft-shelled case on the inside and a hardshell case on the outside. You can find it at:
http://www.musiciansfriend.com/product/Musicians-Friend-Durafoam-ShockResistant-Electric-Guitar-Case?sku=540610
It's a lightweight carrying case that will protect his favorite guitar; yet, it won't make any fuss on his back while carrying it from gig-to-gig! Don't worry about thanking me; every musician needs at least one of these!

As a guitarist, I can say that most are extremely picky when it comes to music supplies. The safest gifts would be to purchase sheet music that you know they would like or a gift certificate. My wife got me a gift certificate for my birthday and I appreciated the thought of her wanting to get something about music for me, yet it gave me the flexibility to buy what I want.

How about an electronic tuner or metronome...
Better yet go to a music store and ask the sales associate.
