Use Tilex or similar product that you would use to clean mold in a bathroom and an old toothbrush.

1. Put on rubber gloves, goggles and face mask when handling mold. 2. Perform the cleaning task outside the house 3. Wipe the mold mildew with some disinfectant wet wipes, discard when it is fully soiled with mold 4. Repeat this until all the molds are cleaned 5. Wipe clean with a new damp cloth 6. Allow it to dry completely in the open air 7. Do not store the shoes in its box where it gets mildewed 8. Store it in an airy place with adequate airflow, avoid keeping it in an closed and humid environment

When I went down to New Orleans to gut houses last February, we walked into some terribly moldy situations. Most of us were wearing some kind of white atheltic shoes and we found that spraying some Clorox Clean-up (a combination of bleach and detergent) and then scrubbing the shoes with a brush, then wiping them down with a wet clean cloth made the shoes wearable again. This would probably not be advisable for colored shoes. Good luck!

Depends upon the type... if they are white, bleach them and wash in the washing machine.
If suede or leather, you're screwed.
To prevent it in the future, make sure your shoes get dry. Moisture is about all mold needs to grow!
